Please note you can only renew on-line if you meet the following conditions:

You agree to pay your premium by Debit/Credit card.
You wish to renew based on the cover and premium details shown on your Renewal Invitation.
Your Renewal Date is not more than 30 days ago.

Please answer the following questions in order to renew your policy

Do you wish to renew based on the cover and premium details shown on your Therapies Insurance Renewal Invitation?

Have you had any claims under this or any similar insurance or had any suits for negligence, error or omission made against you, which would have been the subject of a claim under the insurance being proposed?

Are you aware of any circumstance(s) that may result in any claim or suit being made against you, which would have been the subject of a claim under the insurance being proposed?

Do you understand that you are under a duty to answer all questions posed by the insurer honestly and with reasonable care?

If you now wish to proceed with renewing your policy on line and pay by either debit or credit card, please read the statement below, confirm that you accept its contents and proceed to the on-line Renewal Form.

I understand that the answers I have given will form the basis of my insurance contract, and that any incorrect answer may cause my insurance to be invalid. I have read the Insurance Product Information Document attached to my Therapy Insurance Renewal Invitation
